If you are then able to open the file then you should be able to set up a variable in your workflow to assign a file name, and force that file name to end with.

The usual extension for a word document is. dotx extension then Word will open the document as a template. If you use quick parts the your file will need to be a template to pick up the metadata variables. The document uses a template that uses Quick Parts to pick up metadata from the doc library. I am using a SPD workflow to create an item in a document library. I am trying to do something very similar.
